Insurance Benefits at PCSO
All full-time and part-time members are eligible for medical, dental, vision and life insurance benefits.

Medical Insurance
PCSO will continue to use UnitedHealthcare for medical coverage. Members may choose between two plan options, Platinum and Gold. The Platinum plan provides coverage for acupuncture, infertility treatment and weight loss surgery; the Gold does not. Both plans have the same network of providers, and are open access. Members who choose the Platinum plan will pay higher monthly premiums, and will have lower copays, coinsurance and deductibles than members who choose the Gold plan.

Prescription Drug Coverage
Prescription drug coverage is included with your medical insurance, and is administered for PCSO by OptumRx, a partner with UnitedHealthcare. An easy way to save money is to switch your brand name drug to a less expensive generic drug. Discuss with your physician to see if your prescription drug has a generic alternative that may be right for you. In addition, ask your doctor to write maintenance medication prescriptions for a 3-month supply, if appropriate, so that you can fill through the OptumRx mail-order program and receive the 3-month supply, delivered to your door, for the cost of 2 retail copays. After your second refill of a maintenance medication at a retail pharmacy, you will have to pay an increased copay of 1-1/2 times the retail copay for a 30-day supply.

Dental Coverage
PCSO offers optional dental coverage through Delta Dental. Members may choose between two dental plan options:
  1. Preventive Only plan, or;
  2. Preventive Plus Plan.
Whichever you choose, each plan makes it easy and affordable for you to maintain a healthy smile.
Visit www.deltadentalins.com to find a network provider in your area. Dentists who participate in the Delta Dental Premier network cost less than out of network dentists. Dentists in Delta Dental PPO network will cost even less.

Vision Coverage
PCSO offers optional vision coverage through UnitedHealthcare Vision. The plan covers periodic eye exams, eyeglasses and contact lenses for you and your eligible dependents; in addition, there is a LASIK benefit. Find a network provider and receive the most from your benefits by visiting www.myuhcvision.com. Remember, your UHC medical / Rx card is also your insurance card for vision. You can print a vision ID card by visiting www.myuhcvision.com

Employee Assistance Program
PCSO’s Employee Assistance Program (EAP) services are provided by ComPsych. ComPsych offers confidential services that are tailored to the needs of our members and any person residing in your home, including:
  • Counseling sessions (up to 6 sessions per issue, per plan year, with unlimited issues per year)
  • Helping you and your family cope with family, marital and relationship problems; emotional difficulties such as depression, anxiety and guilt; drug and alcohol dependence
  • Referrals for child and elder care research
  • Assistance with legal and financial concerns
  • 24/7 telephone counseling and referrals by calling 1-888-327-4801
To use the online services, log on to the ComPsych website at www.guidanceresources.com. Click the blue link at bottom right of page that states, “I am a first time user”; enter Company/Organization Web ID: PCSO; create username and password; complete all required fields (those marked with red asterisk); click submit; enter demographics (optional); read terms of use and click inside the check box to indicate your agreement to those terms; click submit. Life Insurance
ACTIVE MEMBER:
Offered through UnitedHealthcare, PCSO provides basic life insurance equal to member’s base annual earnings, rounded up to the nearest $1,000 increment. During open enrollment, members have the option to purchase up to $20,000 in supplemental life insurance without going through medical underwriting, subject to all of the following:

   1.   The additional life insurance must be taken in $5,000 increments;
   2.   The total amount of supplemental life insurance is limited to 5 times  base annual earnings or $250,000 (whichever is less);
   3.   The member was not previously denied for additional life insurance.

During open enrollment, members have the option to purchase additional life insurance for themselves, or their spouse, but a medical Evidence of Insurability questionnaire may be required for underwriting purposes. Do you have a spouse, parent or child who is also employed at the Pinellas County Sheriff's Office? If you answered yes, it is important that you understand that members cannot be both a member and a dependent on any of the agency Life Insurance benefits and, should you need to file a claim, you would NOT receive the "dependent" benefit.

Please remember to keep your life insurance beneficiary information updated! Changes can be made by completing a new beneficiary designation form located on SONET/agencyforms.
