Deputies assigned to the Major Accident Investigation Team (M.A.I.T.) are investigating a fatal three-vehicle crash in Oldsmar.

The crash occurred at about 2:47 p.m. on Wednesday, November 6, 2013.

The crash occurred at the intersection of Tampa Road and St. Petersburg Drive in Oldsmar.

According to investigators 65-year-old Marie Mikuliza was traveling westbound on Tampa Road in the left turn lane approaching St.Petersburg Drive. As she made a left turn onto St. Petersburg Drive, her Cadillac DeVille was struck on the passenger side by two vehicles, a Jeep Liberty driven by 32-year-old Arianis Vevero-Pompa and a Ford Fusion driven by 31-year-old Dustin Keithly, that were traveling eastbound on Tampa Road, side-by-side. Deputies say both drivers applied maximum braking to avoid the collision, but were unsuccessful.

The front passenger seat of the Cadillac DeVille, was occupied by Marie’s husband 80-year-old Joseph Mikuliza. Deputies say Mr. Mikuliza became trapped in the vehicle from the impact. It took Oldsmar Fire Rescue approximately 30 minutes to extricate him. Mikuliza was transported by air to St. Joseph’s Hospital for treatment of life-threatening injuries. He was pronounce deceased at the hospital at 4:17 p.m.. Ms. Mikuliza was also transported to St. Joseph’s for treatment of non-life threatening injuries. The other drivers were not injured in the crash.

At this time it does not appear that speed or alcohol were factors in the crash.

The investigation continues.
